쇼그렌 증후군이란?

쇼그렌 증후군(Sjögren Syndrome, ICD-10: M35.0)은 면역 체계가 침샘과 눈물샘 등 외분비선을 공격하여 구강 건조(xerostomia)와 안구 건조(keratoconjunctivitis sicca)를 유발하는 만성 자가면역 질환입니다. 전 세계 인구의 약 0.1-0.5%에서 발생하며, 여성이 남성보다 9배 이상 호발합니다. 단독으로 발생하는 원발성과, 류마티스 관절염이나 루푸스 등에 동반되는 이차성으로 구분합니다.

원인과 위험 인자

HLA-DR3, HLA-B8 등 유전적 소인에 바이러스 감염(EBV, HCV), 호르몬 변화 등이 복합적으로 작용합니다. 림프구(주로 CD4+ T세포)가 외분비선에 침윤하여 선(gland) 조직을 파괴하고, 항SSA/Ro, 항SSB/La 자가항체가 생성됩니다.
